I strongly resemble that comment...
I'll chime in and note in picture 4, the bar position is much further away from her spine than it should be. Bar should be traveling on a vertical path from the floor up to it's final position as close to the spine as possible.

Level One CF Answer - Core-to-Extremity Violation!
Level Two CF Answer - Bad Deadlift Coach! Coach does 100 penalty burpees and must perform one DangerWOD as penance.
Number one - maybe too much cervical extension and shoulders too far forward, but I think this is the best one.
Number two - shoulders too far behind bar
Number three - T steps in for a frame
Number four - bar too far away from body
